**Responsibilities**

**SUMMARY:** Provides Patient Representative care and services based on the Clinic requirements. Assumes all Patient Representative job responsibilities**.** Verifies established patient information to ensure accuracy and insurance information is correct. Collect co-pay and amounts due if applicable, giving each patient a receipt. Cooperates in cross-training others to ensure coverage at all times.

**QUALIFICATIONS:** To perform this job successfully, an individual must be able to perform each essential function satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. **EDUCATION and/or EXPERIENCE:** High school diploma or general education degree (GED) required. Must possess good written and verbal communication skills, typing, general clerical skills and excellent telephone skills. Medical terminology and one year medical office experience preferred.
