**Patient Care Coordinator**

Our client is an innovative and progressive medical group that has been providing top quality diagnostic and interventional radiology services to the San Diego community for over 40 years.

**Position Summary:**

Patient Care Coordinator position is responsible for the coordination and overall delivery of Interventional Radiology patient care. The position is responsible for assisting the radiologist with patient assessment and continuity of care before, during and after various radiology exams involving interventional diagnostic procedures. Day-to-day responsibilities require good use of judgment regarding timely and appropriate communication to team members and families.

**Responsibilities:**

* Greets patients and visitors as they enter the hospital, notify the appropriate person of the individual's arrival.

* Prepares carts with medical instruments and supplies as required by the Physician or Physician Assistant.

* Perform general office responsibilities to include typing, answering telephone, responding to voicemail and client inquiries in a timely manner, preparing outgoing mail and ensuring patient charts are organized and located in proper place.

* Transcribes information regarding patient visits for medical reports.

* Responsible for inventory, ordering supplies as needed, rotating stock to avoid product expiration and packaging and labeling medical supplies.

* Responsible for scheduling consults and IR procedures. Ensure appointment hours are filled in accordance with appointment scheduling guidelines by reviewing patient schedule weekly. Maintain telephone call list to notify patients of openings or changes in appointment times and schedules accordingly.

* Combine new patient records and charts and maintains up-to-date patient records as required for physicians and billing purposes.

* Review procedures with patients, including patient medications and ensures prescriptions for patients are refilled or called in and that disability or family leave paper work is filled out.

* Ensure that all procedures are scheduled properly, orders are written, labs are current, imaging is available where procedures will be done, authorization already obtained and all special equipment is ordered.

**Requirements:**

* Must have knowledge of Microsoft Word; Microsoft Outlook and Microsoft Excel.

* Adheres to HIPPA requirements. Ensure the security of all Private Health Information (PHI) by following adopted office procedures.

* High school diploma or general education degree (GED)

* CPR certification

* Two years related experience and/or training; or equivalent combination of education and healthcare experience.
