About the Position:

• Looking for an Adult Geri NP or PA in San Rafael, CA
• A minimum of 3 years of primary care clinical experience is required.
• Provides care to patients aged 18 to 100+.
• Responsible for delivering and managing direct patient care, including physical exams, evaluations, diagnoses, and treatments for the assigned patient population.
• Prescribes medications and treatment plans based on assessed medical conditions.
• Refers patients to specialists and collaborates with other healthcare providers as needed.
• Adheres to established departmental policies, procedures, and objectives.
• Maintains professional relationships with both staff and patients.
• Performs additional tasks aligned with clinic goals and objectives as directed by the Medical Director.
• Participates in care team responsibilities as outlined in the Care Team Patient-Centered Medical Home Procedures.
• Completes other duties as assigned.

About the Compensation & Benefits:

• Signing bonus of up to $50,000 for eligible applicants (specific program terms apply).
• Competitive salary range of $135,000 $160,000 annually.
