Established in 2023, CommuniCare+OLE is the result of a union of two health centers with a deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. JOB SUMMARY/OVERVIEW: The Peer Support Advocate (Mobile Medicine) reports directly to the Community and Migrant Health Operations Manager and aims to reduce risk associated with homelessness, use of substances and struggles with addiction. The Peer Support Advocate will empower clients through strengths-based coaching supporting linkages to community resources and educating beneficiaries and their families using the harm reduction model of care.

MINIMUM POSITION REQUIREMENTS: EDUCATION, EXPERIENCE, SKILLS/TRAINING
- Education:
High school diploma or equivalent required
- Experience/Lived Experience:
The Peer Advocate (Mobile Medicine) uses personal/lived experience (identifying as a person in recovery from substance use/addiction and/or homelessness and/or mental illness) to provide support to individuals.
- Special Skills/Training:
- Cal MHSA Peer Support Training and/or Certification (preferred)
- Bilingual Spanish/English preferred.
- BLS Certification required
- Drivers License required.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
1.
Assist the Mobile Medicine Team in working with patients with SUD and/or mild to moderate mental health issues and/or homelessness by providing support and advocacy, encouragement and hope in a non-judgmental way. Supporting linkages to care to empower participants including family members and significant others.
2.
Assist patients in preparing for health care/treatment visits and appointments and managing complex health systems.
3.
Complete necessary documentation and tracking in a timely manner
4.
Collaborate with other CommuniCare+OLE programs and outside agencies to assist in the appropriate transfer of care and follow-up services when needed.
5.
Responsible for utilizing tracking systems including scheduling follow-up appointments, scheduling appointments, assisting with referrals, discharge planning, follow ups.
6.
Identify and assist in promoting wellness and recovery by assisting in setting goals, action steps and achievement of goals
7.
Assist in daily management of tasks, such as attending appointments, counseling sessions etc.
8.
Assist in problem solving to reduce crises.
Provide social and emotional support and feedback and follow up with clients.
9.
Develop approved plans of care with the patient.
10.
Provide culturally competent individual services promoting recovery, resiliency, engagement, socialization, self-sufficiency, self-advocacy and the developments of natural supports.
11.
Utilize harm reduction practices to support engagement and retention of patients.
12.
Support strategies that are focused on lowering barriers to care to improve engagement and retention
13.
Performs other duties as assigned.
