The Discharge/Eviction Specialist is responsible for reviewing skilled nursing facility discharge notices and facility eviction notices (both skilled and Retirement Communities for the Elderly) to ensure that residents' rights are being honored. The notices are reviewed to ensure that the required information on residents' rights is included in the notice and that the contact information for the local long-term care Ombudsman program is current and the appeal process information is stated. The specialist will work with facility contacts to resolve any resident rights issues. The Specialist will work with the Intake Team and the Ombudsman Program on identifying inappropriate discharge practices. Any inappropriate discharges will be brought the attention of the assigned Ombudsman.
In addition to the review of facility notifications, the specialist may be asked to work with residents/family members and facility staff to ensure that a safe discharge plan is in place.
