Job Description

We are looking for a Social Services Case Manager to join our team of Service Coordinators. In this role you will manage the delivery of services to NBRC clients, including those with intensive physical or emotional needs, ensuring that the clients Individual Program Plan and its objectives are pursued. NBRC employees promote opportunities by supporting choices for people with developmental disabilities.

Responsibilities
  • Monitor vendor services and verify that services are delivered in accordance with purchase of service authorizations
  • Evaluate the effectiveness of clients Individual Program Plans and ensure appropriate community resources are available
  • Conduct annual reassessments of clients developmental status with the interdisciplinary team
  • Conduct client assessments on an occasional basis
Qualifications
  • Bachelors Degree in a field related to developmental disabilities (e.g., Social Work, Psychology, Counseling, Special Education, Nursing). Minimum education requirement may not be waived.
  • Masters Degree preferred
  • 3 years experience working with individuals with developmental disabilities
  • Bilingual language ability (Spanish to English) is required
Remote/Hybrid Work Note

All employment at NBRC requires face-to-face, in-person interactions with clients, families, community, and co-workers. Candidates must live within commutable distance to their preferred NBRC office and assigned clients. 100% remote work is not available; a hybrid arrangement may be possible after successful in-office performance for at least 3 months.

Salary

$60,732 $66,984 with opportunities for 5% annual increases.
