Program Description
IHART is a mobile community resource team comprised of non-police personnel that will provide timely responses - with a targeted response time of 10-12 minutes in the city of Vallejo. IHART will provide welfare checks, screening, and assessments, providing proactive, easily accessible, culturally responsive engagement to link residents to resources based on their specific and urgent mental and behavioral health needs. IHART mobile community resource unit will provide trauma-informed response and linkages for behavioral health and social service-related calls, thus reducing the role of police and improving outcomes for individuals experiencing mental health- and social services-related crises. IHART will have partnerships with community system partners, to include, but not limited to, hospital emergency departments, Fire/EMS departments, SCBDHS' mobile crisis team to provide 12-hour, 7 days a week alternative response services to low-level 911 calls from Vallejo Residents. IHART response will focus on de-escalation, mitigation, and prevention of repeated emergency situations. Implementation of the IHART Program will build a collaborative network of service providers to allow for a safer response to behavioral health, quality of life and low threshold medical calls. Data collection activities from the IHART Program will support opportunities for evaluation and inform strategies for improvement.
Position Summary
Case Manager equivalent to Navigator will adhere to the principles of Motivational Interviewing, Harm Reduction, Trauma-informed care, Cognitive Behavioral Therapy, Thinking For Change, and Recovery-and-Wellness in order to engage high-need adult and TAY probation in therapeutic interventions to help identify problems that can have regular occurrences and vary in severity and frequency, thus linking residents to the appropriate social and behavioral services and supporting them on their path to wellness and recovery.
In partnership with a Community Responder/Community Intervention Worker, the Case Manager/Navigator will engage Vallejo residents in areas they are most comfortable in the community. The Case Manager/Navigator will perform comprehensive assessments, establish Individual Care Plans to guide service provision, and develop crisis prevention and intervention plans. These activities may include coordinating mental health, social work, resident advocacy, educational, healthcare, vocational, housing, transportation, respite care, and recreational services, facilitating groups/workshops in the community. Case Manager/Navigator will adhere to laws regarding confidentiality and reporting requirements, and will maintain accurate, detailed observation and progress records for electronic data entry. Additional duties may be assigned as needed to meet the requirements of the program.
Program Specific Responsibilities
• Works a flexible, part-time schedule to accommodate program/residents' needs. Some evening and weekend hours are required.
• Performs intake and comprehensive assessments with adults and TAY who may have dual diagnosis disabilities (substance use disorders).
• Provide case management and navigation services including evaluations, service plan development, assisting residents with sustainable housing, financial, medical, and other needed services. Coordinate with other service providers.
• Provides crisis prevention planning and crisis intervention and support, as appropriate.
• Conduct assertive community outreach and engagement to safely provide services to residents within the community.
• Maintain documentation after each resident contact or service, in accordance with Felton Institute's mandated standards and VPD requirements.
• Contribute to team meetings and case conferences through presentation of cases, discussion of behavioral health issues, and exchange of information and ideas.
• Other duties and responsibilities may be assigned as required to meet program goals and objectives.
Minimum Qualifications
• A minimum of 2 years of case management and navigation experience with an adult & TAY population--and substance abuse experience interventions preferred.
• Minimum of 1 year CBT and DBT intervention experience, applying comprehensive range of case management and substance abuse counseling, assessment and crisis intervention skills
• Bachelor's degree in social work, Counseling, Criminal Justice or Psychology preferred
• Bi-lingual/Bi-cultural Spanish-speaking clinicians encouraged to apply
• Ability to work cooperatively on a multidisciplinary team, and to work with residents of diverse ethnic and cultural backgrounds
• Ability to develop and implement an Assertive Community Treatment, ACT and Sequential Intercept Model, SIM that includes service linkage, as well as intervention and personal change goals
• Working knowledge of a variety of computer software applications (i.e., Microsoft Word, Excel, Access, PowerPoint, Adobe Acrobat)
• Strong organizational and time-management skills
• Strong communication (both verbal and written)
• Interpersonal skills and ability to prioritize projects and follow through
• Ability to maintain proper documentation as determined by programmatic requirements
• Ability to be a team player with a welcoming attitude

Salary Range: $35 per hour
