The Community Liaison is responsible for developing and maintaining positive relationships with referral sources to increase community awareness and referrals for Home Health, Hospice, Home Infusion, PACE, Transitional Care Units, and other services within assigned accounts. This role works with discharge planners, facility staff, physicians, and patients to assess service appropriateness, coordinate care transitions, and ensure documentation meets agency and regulatory standards. The Community Liaison also provides education on home-based care, end-of-life care, pain and symptom management, and serves as a clinical resource and educator for physicians, facility staff, patients, families, and community partners.
