Position Responsibilities:

  1. Direct Care and Interventions (35%): Provide caring support to a caseload of youth through in-person visits and virtual communications. Assess and ensure youth well-being and safety, develop individualized plans, and mentor youth towards self-sufficiency.
  2. Case Management/Coordination (35%): Collaborate with community service providers, maintaining open communication and targeted support for education, employment, and housing needs.
  3. Documentation/Administrative Duties (30%): Complete necessary documentation such as care plans and progress notes while participating in training and team meetings.

Qualifications: A Bachelor's degree in social work, psychology, counseling, or a related field is required, along with a minimum of two years experience in child welfare or with transition-age youth. A Master's degree is preferred.

Strong communication skills and the ability to work collaboratively are essential. A valid CA driver's license, car access, and proficiency in Microsoft Office applications are also required.

Availability: Flexible schedule, with the possibility of working up to 29 hours a week based on organizational needs.

Compensation: $32.16 - $34.29 Hourly Wage
