Job Description: We are currently seeking dedicated healthcare professionals to join our team. As a member of Mare Island Home Health, you will have the opportunity to make a profound difference in the lives of our patients and veterans by providing comprehensive care tailored to their unique needs. Positions available include:

• Registered Nurse (RN)
• Licensed Vocational Nurse (LVN)
• Home Health Aide (HHA)
• Physical Therapist (PT)
• Physical Therapist Assistant (PTA)
• Occupational Therapist (OT)
• Certified Occupational Therapy Assistant (COTA)
• Speech Therapist (ST)
• Medical Social Worker (MSW)

Responsibilities:

• Conduct thorough assessments and develop individualized care plans.
• Provide skilled nursing care, therapy services, or social work support according to prescribed treatment plans
• Educate patients and caregivers on disease prevention and management.
• Collaborate with interdisciplinary team members to ensure holistic patient care.
• Document all patient care services accurately and timely.

Initial Requirements:

• Valid California state licensure or certification in your respective field (RN, LVN, PT, PTA, OT, COTA, ST, MSW).
• Current CPR certification
• Current CA driver's license plus proof of car insurance.
• Minimum one year ST clinical experience, (two+ years is more preferable)
• Computer/Internet experience (online charting to be used)
• Bi/Multi-lingual skills a plus (e.g. Spanish, Russian, Tagalog, etc.)

• Ability to travel to patient homes across Solano County, Napa County, and Contra Costa County.
