Responsibilities
- Perform initial and ongoing physical therapy assessments for residents in a SNF/short-term rehab setting
- Develop, implement, and monitor individualized treatment plans to improve mobility, strength, balance, and functional independence
- Provide therapeutic interventions such as gait training, therapeutic exercises, manual therapy, and modalities as appropriate
- Document patient progress accurately in charting and communicate progress and any changes to the interdisciplinary team
- Educate patients and families on home programs, safety, and fall prevention; ensure patient safety and infection control during sessions
- Collaborate with nursing, therapy aides, and other clinicians to support discharge planning and continuity of care

Requirements
- SNF experience preferred
- Ability to work in a skilled nursing/short-term rehab environment
- Flexibility to float between two nearby facilities within walking distance in Napa, CA
- Strong communication and documentation skills
- Must be able to meet the physical demands of a 40-hour workweek and adapt to a flexible schedule
