Job Summary:

The Registered Nurse provides skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. This role involves patient assessments, medication administration, treatment planning, and collaboration with an interdisciplinary team. Strong communication and collaboration with patients, providers, and team members are essential.

Location:Santa Rosa, California, United States

Responsibilities:

- Provide skilled nursing care.
- Perform patient assessments.
- Administer medications.
- Develop and implement treatment plans.
- Collaborate with the interdisciplinary team.
- Provide case management.
- Provide hospice care.
- Collaborate with patients, providers, and other team members.

Required Skills & Certifications:

- Active Registered Nurse license in the state of working.
- Clinical Assessment, Medication Administration, and Treatment Planning skills.
- Minimum 3 years of experience.
- Bachelor's degree in Nursing or related field.
- Driver's license, car insurance, and automobile for field work.

Preferred Skills & Certifications:

- Acute rehab experience.

Special Considerations:

- Must be willing to float between like units.
- On-call may be required.
- RTO must be included at the time of submission.

Scheduling:
- Schedule could be Tuesday through Saturday or Sunday through Thursday.
- Block scheduling is per manager discretion.
